IMPORTANT:
Data entered on Web (i.e., HTML) versions of forms currently can only be printed out in hard copy, but cannot be saved to the hard drive of the filer's computer. For PDF versions of the OSC-11, OSC-12, and OSC-13 forms all data entered can be saved for later retrieval when using Acrobat Reader version 5.05 or higher. Web (HTML) versions permit data entered into multi-line fields to scroll beyond the initial field size. Only data displayed within the initial field area will be viewable on printed hard copy.
